Filling out a green application for a visa to the Czech Republic. To obtain a long-term residence permit in the Czech Republic, you will additionally need a green application form for a residence permit in the Czech Republic
Take the questionnaire at any department of the Ministry of Internal Affairs of the Czech Republic or download from the link

The questionnaire is completed in blue ink, only by hand and in Czech. There should be no corrections and errors, carefully check everything and, if you make a mistake, rewrite it again. You should fill out the black application form for a long-term visa with a pen, and when applying for a residence permit in the Czech Republic, also a green application form in the Czech Republic.

At the very top of the green questionnaire, we tick off the purpose of submitting documents:
Necessarily. If you had an ordinary long-term visa pasted into your passport, then you check the box o povolení k dlouhodobému pobytu. If you are submitting the green application for the first time, check the second box at the top of the green visa application form for the Czech Republic for study.

3. Your previous last names. Item 3 – only for those who changed their last name. It is necessary to write the previous surname in Latin letters.

Your last completed education (for example, you graduated from high school, then you write – středoškolské vzdělání, higher education – vysokoškolské vzdělání) Item 11 – schoolchildren and college students fill in according to the model, if they have a certificate – středoškolské, university students – vysokoškolské neúplné , with a university diploma – vysokoškolské
12. Profession / position (for example, student – student / studentka, entrepreneur – podnikatel / ka)
13. Your last place of work / study before coming to the Czech Republic – not required, here is an example:

14. Your place of work / study after arriving in the Czech Republic – fill in the data of your future place of study (i.e. university)
15. Purpose of stay – students studium. The purpose of the stay – studium or ostatní, depends on your university.
16. Last address abroad – write your registration at the place of citizenship (street, house number, city, index)
17. Your last stay in the Czech Republic for more than 3 months –
Where “FROM” – write the date of issue of your last long-term visa / biometrics (for example, 09/01/2022)
Where “TO” – write the expiration date of your last visa / biometrics (for example, 08/31/2023)
below – the purpose and place of stay – for example, studium Praha
18. Address of residence in the Czech Republic (street, house number, city, postcode)
19. Correspondence address – postal code, if different from residence. Most often, this column is not filled out so that the police do not have unnecessary questions.
20. Day of arrival in the Czech Republic – not filled in for an extension.

Advice. There should be no mistakes and corrections in the questionnaire, if you make a mistake, then you can use a white corrector (preferably not liquid) and write the correct data on top. Questionnaire with crossed-out data will not be accepted.
