Documents for enrollment in LF UK students
Important information for those admitted to 1. LF. Enrollment in LF UK students.
Registration for students in the medical faculty of Charles University. What you need to have with you:
– passport/visa or residence permit
– one photo 3.5×4.5 – write your last name, first name on the back
– certified copy of the nostrification
– a certified copy of the ZKLF exam at C1 medical level
– medical certificate

– hepatitis B certificate

What should I do if I am missing at least one document from the list?
Write a request to postpone the “enrollment day for LF UK students”.

Without at least one of the documents, you will not be allowed to enroll in students.

Where can I sign a medical certificate for the faculty?
Contact your insurance company, explain what you need and ask for a doctor who can sign this certificate for you. Most likely, this will be paid, because insurance companies usually do not cover medical certificates (for the reasons that you personally need it and this is not a matter of life and death)

Prague medicine entry requirements. Where can I sign the hepatitis B certificate?
If you already have a hepatitis B vaccine, then you don’t need to vaccinate it again! It is enough to turn to praktickému lékaři, who will write you a referral for blood donation, which will confirm the presence of antibodies in your blood – this will be the evidence, on the basis of which you will be signed a document on hepatitis B.
If you do not have this vaccine, go to the doctor again, get a prescription for the vaccine, buy it at the pharmacy and put it in the hospital.
Enrollment in LF UK students.Fill out the form only.
Hepatitis B vaccine – three doses, a break between 1 and 3 doses of 6 months, make sure to have time to deliver all 3 doses by March 31 of the school year – this is the last opportunity to convey a certificate that you are fully vaccinated against hepatitis B.
